Annual Wellness Visit (AWV)
What is Annual Wellness Visit?
Eligibility Requirements:
Patients must be enrolled in Medicare Part B for at least 12 months
AWVs are offered once annually for eligible patients.
The visit must include a Health Risk Assessment (HRA) and other preventive care elements.
Initial AWV with HRA.
CPT Code: G0438
Once per patient's lifetime.
Completion of HRA, identification of risk factors, and creation of a personalized prevention plan.
Subsequent AWVS.
CPT Code: G0439
Annually.
Updates to the HRA, adjustments to the prevention plan, and ongoing risk factor analysis.
