Additional 20 minutes of non-complex CCM.

Up to 2x per month.

60 minutes of complex CCM.

Monthly
Logs of care plan revisions, time spent addressing complexities, and measurable outcomes.
Additional 30 minutes of complex CCM.

Up to 2x per month.
Records of extended care activities, time logs, and documentation of outcomes.
